On to the next BFF, the next set of adventurers, with hardly a word or nod to those who went before.
It's the number one source (among many) of largely unexplored darkness in the Doctor's character - if it does appear, it often winds up overdone in fanfic.
That said, I really, really didn't intend for Romana and K9 to be there in Winter, because I was consciously trying to avoid the Winter-as-an-ending archetype. Originally, it was just supposed to be the outsider's view of the Sarah/Four/Harry TARDIS team accidentally landing there again and then taking off to the next adventure - but the K9 and Romana dialogue got stuck in my head and I had about half an hour to finish the fic and edit it and post it, so I just went with it. Harry's little introspective moment at the end of Autumn was also unexpected (it was just supposed to be Sarah thinking along the same lines) - and the two changes fit together nicely. I'm really happy with how it turned out. :D
Would I? I don't know.
It really is a difficult question. The argument, of course, is that you come back to Earth with new eyes, and you live a different life. I'm always fascinated by which companions part voluntarily from the Doctor to have their own adventures.
